脳腫瘍によるものは少ない.今回, 筆者は神経膠腫のため左前頭葉を切除した9歳の1小児例(K.S.)を経験し, 手術後3カ月時(9歳4カ月)から12カ月時(10歳)にかけて種々の言語検査を行うことができた.その結果, 明らかな失語症状は示さず, 脳腫瘍による言語症状は頭部外傷などとは別に扱った方がよいと思われる結果を得た.症例の経過および諸検査の結果は以下の通りであった.1)手術後3日目から発話が認められた.2)術後3カ月時(9歳4カ月)のWISC-R知能検査の結果は, 言語性IQ85,動作性IQ82,全IQ82であった.術後4カ月時(9歳5カ月)のKohs検査によるPIQは192であった.3)語い力は, 術後7カ月(9歳8カ月)時点では年齢相当レベルの語い年齢(10歳3カ月)であった.また, 対面呼称にも問題はなかった.4)理解力・文法能力ともに検査上問題は認められず, 助詞の使用も適切で日常生活に支障はなかった.5)発話能力に問題はなく, 構音障害も認められず, 流暢な発話であった.6)文字言語力については, 音読・読解に問題はなかったが, 漢字の書字には時に誤りが認められ.この傾向は術後12カ月時(10歳)でも見られた.7)加減算・九九に問題はなく, 3桁÷2桁の計算も術後7カ月(9歳8カ月)で可能となった.8)聴覚的記銘力は数字で5桁, 物品名で3つとやや劣っていたが, 視覚記銘力は正常だった.9)術後性格的にはおとなしくなり, 声も小さく無口になった.また集中力も低下した.学業成績は病前の「上の下」から「中の下」に低下した.国語・算数の文章題がやや苦手となった.10)本例は10歳8カ月時に死亡した.以上の経過と検査結果から, 本例に失語症はないと思われた.また, 非言語性知能に低下が認められなかったことから, 本例の言語機能は左半球内で再編成されたものと思われた.本例は新しい課題の学習にやや困難を示したが, これは記銘力の低下によるものと推測された.脳機能を解明するためには,
